  Good question! I was watching breaking bad doing my thing.. Keep puffing along. And I was like wtf is that hole forming? I got curious and kept puffing it only got worse.. So I wanted to see how bad it could get. Then it turned into really hot... Then it got chunked! 
  
  normally I encounter plugged or side burning I can correct.. This one had no hope.
